DISTINGUISHING FEATURES OF THE CLASS

The work involves responsibility for performing outreach and referral services for programs of the Onondaga County Health Department. An employee in this class is responsible for informing and educating participants concerning program policies and goals. General supervision is received from an administrative superior who reviews the work of the incumbent with regard to program and clients. Supervisory responsibilities are not typically a function of this position. Certain assignments made to employees in this class will require reasonable access to transportation to meet field work requirements made in the ordinary course of business in a timely and efficient manner. Does related work as required.

TYPICAL WORK ACTIVITIES

Performs outreach and referral services in targeted at risk census areas for programs of the Onondaga County Health Department.

Attends health and/or medical clinics, programs and meetings to inform and educate participants regarding program policies and goals.

Conducts or assists with group education sessions.

Distributes literature and program materials.

Assists participants with obtaining transportation, day care and medical services.

Acts as referral agent for patients to other Onondaga County Health Department programs and services; answers patients questions; assists patients in participating in programs and services.

Conducts patient follow-up including, but not limited to, telephone contact and home visits.

Conducts surveys with selected minority organizations; assists with data

collection and compilation.

Assists participants with filling out appropriate paper work.

Participates in public health preparedness activities as trained and assigned.

Utilizes appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial and ethnic backgrounds, sexual orientations, lifestyles and physical abilities.

FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS

Good knowledge of the policies, priorities and goals of the Onondaga County Health Department programs to which assigned.

Working knowledge of local agencies or programs that provide transportation, day care or medical services.

Ability to prepare simple narrative and numerical reports.

Ability to establish effective working relationships with health professionals, nurses and program participants.

Ability to follow oral and written instructions.

Ability to understand and interpret written material.
